  • CSS @keyframes animation automatically hides hover/focus-triggered content (e.g., fade out), violating the persistent requirement.
  • Elements with aria-haspopup trigger popup content but lack a keyboard dismiss mechanism (e.g., Escape key handler).
  • Form or form element automatically submits when focused, without explicit user action.
  • Interactive element lacks an accessible name, making its purpose unclear to screen reader users.
